Job Details
- BLS certification required for all direct patient care personnel.
- Fit Test must be completed prior to onboarding.
- Trinity 9 Panel Drug Screen must be completed within 30 days of start date, preferred within 14 days.
- Trinity Code of Conduct Form must be signed and on file.
- Trinity I9 must be completed by the agency.
- Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed and on file.
- Trinity Integrity and Audit Services Form must be completed.
- Trinity Specialty Specific Competency Testing must be completed within a year.
- Trinity Color Vision must be completed.
- 2 Step PPD OR IGRA (Quantiferon/Tspot) must be completed.
- Trinity Specific Covid Questionnaire must be uploaded once completed.
- Trinity Skills Checklist must be provided for clinical contractors.
- COVID-19 Vaccine proof required.
- MMR vaccination proof required.
- Hepatitis B vaccination proof required.
- Influenza Vaccine proof required.
- Varicella vaccination proof required.
- Tdap vaccination proof required.
- Physical/Health Statement must be provided.
- Resume and Skills Checklist within a year, along with 2 Professional References are required.
Job Requirements
- All engaged contractors must have page 27 signed form on file in Trinity VMS.
- Federal, Nationwide, County Criminal background check required within 30 days of start.
