Job Details
- BLS certification required for all direct patient care personnel
- Trinity 9 Panel Drug Screen must be completed within 30 days of start date
- Trinity Code of Conduct Form must be signed and on file
- Trinity I9 must be completed by the agency
- Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed
- Trinity Integrity and Audit Services Form must be completed
- Trinity Specialty Specific Competency Testing must be completed within a year
- Trinity Color Vision must be completed
- 2 Step PPD OR IGRA (Quantiferon/Tspot) must be completed
- Trinity Specific Covid Questionnaire must be uploaded
- Trinity Skills Checklist must be completed
- COVID-19 Vaccine proof required
- MMR vaccination proof required
- Hepatitis B vaccination proof required
- Influenza Vaccine proof required
- Varicella vaccination proof required
- Tdap vaccination proof required
- Physical/Health Statement must be provided
- Resume and Skills Checklist within a year required
- Trinity Profile must be submitted
- Trinity Conflict Of Interest Disclosure Form must be signed
- Trinity Criminal Background Check must be completed
- Trinity Education Verification must be attached
- Trinity EPLS report must be attached
- Trinity GSA/OIG report must be attached
- Trinity NSO report must be attached
- Trinity System Access Form must be completed
