Job Details
The Nurse Practitioner (NP) works in the Ambulatory HIV Medicine Clinic (IDC/Immunodeficiency Center) and the Pride Clinic (LGBTQAI+ primary care) to provide comprehensive, patient-centered care. This role focuses on the prevention, diagnosis, and management of HIV and relatedconditions in an outpatient setting as well as providing comprehensive sexual health services and gender affirming primary care. The NP works collaboratively with physicians, nurses, social workers, and other interdisciplinary team members to deliver high-quality care, promote adherence to treatment, and improve long-term health outcomes. The ideal candidate is experienced in infectious disease or HIV care, gender affirming care, culturally competent, and committed to serving diverse and often underserved populations.
Job Description
Key Responsibilities
- Provide primary and specialized care to LGBTQAI+ patients or those patients living with HIV in an outpatient clinic setting
- Perform comprehensive health assessments, including medical history, physical exams, and risk evaluations
- Diagnose and manage HIV infection and related comorbidities, including opportunistic infections
- Prescribe and manage antiretroviral therapy (ART) and monitor treatment adherence and effectiveness
- Prescribe and manage pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) for individuals exposed to or vulnerable to contracting HIV
- Prescribe and manage gender affirming hormone therapy (GAHT)
- Order, interpret, and follow up on diagnostic tests and laboratory results
- Provide preventive care, including vaccinations, STI screening, and comprehensive sexual health counseling
- Educate patients on disease management, medication adherence, and lifestyle modifications
- Collaborate with interdisciplinary team members, including case managers, pharmacists, and behavioral health providers
- Participate in quality improvement initiatives and maintain accurate, timely documentation in the electronic health record (EHR)
- Stay current with evolving treatment guidelines and best practices
