Sunrise Senior Living - - Responsibilities: Deliver care and services for designated group of guests/residents according to Individualized Service Plans (ISP).; Report all incidents immediately to appropriate supervisor / team member.; Participate in the care planning for their designated guests/residents; Perform individualized personal care and activities of daily living (ADL); documents ADL in flow sheets; Partner with licensed nurse to plan, develop, organize, provide and execute individualized restorative therapy programs, activities and standards for their designated guests/residents.
