Suncrest Hospice - - Responsibilities: The Home Health Aide position works under the direction of a Case Manager/DON and provides resident care to assigned residents in accordance with resident assessments and care plans.; Coordinate proper types of care with licensed care staff and follow nursing policies and safety guidelines.; Collaborate with nurses, social workers, chaplains, volunteers, and HHA leadership as part of the patient care team.
