Home Care Evolution - - Responsibilities: Hands-On ADL Support: Safely assist transfers, bathing, grooming, toileting, and mobilityalways protecting dignity; Home Environment & Infection Control: Change linens, do light laundry, and maintain a hazard-free, sanitized space; Medication & Symptom Monitoring: Provide reminders, observe for changes, and document vitals or red-flag signs for your supervising nurse; Therapeutic Engagement: Read aloud, reminisce, guide range-of-motion exercises, or accompany short walks; Accurate Documentation: Record care notes, progress, and concerns in our digital charting system before each shift ends
